Salary Range: Based on Experience

Overview

The Day Services Caregiver provides compassionate, person-centered support to individuals with disabilities in a day program setting. This role focuses on assisting participants with daily living activities, implementing Person Centered Plans (PCPs), promoting skill development, community integration and fostering an inclusive and engaging environment. The Caregiver is responsible for ensuring the safety, health, and well-being of all participants during program hours.

Key Responsibilities:

  • Assist participants with activities of daily living (ADLs), including, but not limited to, assisting or providing prompting with personal hygiene, toileting, dressing, feeding, and mobility assistance, as outlined in their Person Centered Plan (PCP).
  • Provide medication administration as delegated in the Medication Administration Record (MAR) following established procedures and documenting accurately.
  • Monitor participants' health and physical condition, promptly report any changes, concerns, or incidents to the Day Service Coordinator/Supervisor.
  • Assist with safe transfers, lifting, and repositioning using appropriate techniques and adaptive equipment (e.g., walkers,wheelchairs).
  • Ensure the safety and cleanliness of the program environment.

Program Implementation and Engagement

  • Implement daily program schedules and activities designed to meet the goals outlined in the participants' PCPs.
  • Facilitate engaging activities that promote social interaction, communication, cognitive stimulation, educational activity, physical activity and skill development (e.g., arts and crafts, community outings, ADLs, games).
  • Actively encourage participants' independence and choice in all aspects of the program.
  • Support participants during community integration activities, ensuring safety and appropriate behavior.
  • Document participant progress, behavior, and daily activities accurately and timely.

Community Integration and Socialization

  • Facilitate client participation in social, recreational, educational, and vocational activities.
  • Provide transportation for community outings, and errands using the client's or personal vehicle as arranged.
  • Encourage the development of meaningful relationships and community connections.
  • Act as a positive role model and advocate for the client.

Transportation Services

  • Pick up participants in the community and bring them to the Day Service or home as scheduled.

Documentation and Communication

  • Maintain accurate, timely, and thorough documentation of daily activities, progress notes, incident reports, medication logs, and financial expenditures.
  • Communicate effectively and professionally with supervisors, clients, and co-workers.
  • Refer family members and guardians to on-call supervisor or Coordinator for medical questions or other concerns.
  • Report any issues or concerns to the assigned coordinator or on-call supervisor.
  • Attend all mandatory staff meetings and training sessions.
 

Other Duties 

  • Complete all scheduled daily tasks, seeking clarification from the Day Services Supervisor.
  • Ensure all company vehicles are maintained, clean and fueled.
  • Other duties as assigned by the Day Services Supervisor or Director of Residential Services.

Qualifications

  • Must be at least 18 years of age.
  • Must have and maintain a valid driver’s license in good standing and be able to be an approved driver under the company’s liability insurance coverage.
  • Must pass a comprehensive background check (local, state, and federal) and drug screening.
  • Must have a minimum of a high school diploma or equivalent.
  • Must be able to give and receive feedback in a professional manner.
  • Must be proficient in reading, writing and speaking English.

Working Conditions and Physical Demands

  • Must be able to lift at least 50lbs, bend and twist.
  • Must be able to sit for extended periods in a home environment or in a vehicle.
  • Must be able to traverse in home, office and community settings.
